Develops the overall outreach strategy to increase program participation and insurance enrollment among community members. This individual will be supervising three to five community outreach staff and will work to increase insurance enrollment among uninsured Will County Community Health Center (WCCHC) patients and the Will County Community.

Under the direction of the Program Coordinator of Epidemiology and Communicable Disease, provide overall management and support for a unified congregate response to Healthcare Acquired Infections and Multi-Drug Resistant Organisms as required by IDPH.
The PHM is responsible for serving as a facilitator for assuring optimal and timely disease management and prevention efforts, develop and implement systems to engage patients in their healthcare through care coordination, care management, outreach, case management, and direct efforts in managing identified chronic care or resource intensive patients, engage physician and staff to facilitate completion of incentive-based care.
