About HealthWorks of Illinois

Will County Health Department is the Lead Agency for Will and Grundy counties for HealthWorks of Illinois. HealthWorks of Illinois is a medical case management program for children under age 21 who are in the custody of Illinois Department of Children and Family Services. HealthWorks maintains a network of providers for Initial Health Screenings, Comprehensive Health Examinations, primary care physicians and specialty care physicians

Medical Case Manager

Every child that enters DCFS custody is assigned to a medical case manager (MCM). Foster children age 0-6 receive medical case management services up to age 6. Children over the age of 6 receive medical case management services for the first 45 days of DCFS custody.

The MCM assists the foster parent in choosing a primary care physician and ensuring that all the necessary exams are scheduled, including the Comprehensive Health Evaluation. The MCM is responsible to obtain past medical history and forward that to the Primary Care Physician.

MCM also maintains regular contact with the foster parent to answer any medical questions and receive an update on any medical visits and forward that to the DCFS/POS case worker.
