INTAKE, ORIENTATION, REGISTRATION

WILL COUNTY HEALTH DEPARTMENT

Intake/Orientation Groups are provided for new consumers requesting services through Central Intake

Staff shall ensure prompt, nondiscriminatory access to services. The Orientation Group guides the consumer to appropriate services by evaluating your needs. You then will be scheduled for a first appointment with a clinician and/or provided a referral for needed services.

During Group the facilitating clinician explains and provides the following documents at the start of group:

  • Consumer Handbook
  • Physician Health History
  • Releases of Information
  • Referrals to assist consumer
  • Primary Care Coordination

The facilitating clinician will review your rights, the agency rights and confidentiality.

Orientation Groups

Orientation Groups are provided for both Adults and Children requesting services. The Groups are held twice a week. Consumers are scheduled by contacting the Intake Office at 815-727-8521 (press 1 for Adult or press 2 for Child and Adolescent)

Child & Adolescent Orientation Groups
Monday 10:00-11:00am
Thursday 3:00-4:00pm

Adult Orientation Groups
Tuesday 2:00-3:00pm
Friday 10:00-11:00am

Please bring a governmental photo ID, proof of income, and insurance card to the appointment.

After group you will meet with a reimbursement specialist to complete registration and make your first appointment with you clinician.
