Our Patient Portal allows you online access to your personal health record. The program also allows you to:

  • View your test results once your provider has reviewed and released them

  • Send non-urgent messages to your care team

  • Request medication refills

  • Request appointments. If it is your first time on the site, click the blue bar in the box on the top right that reads “I am new here.” You will then be able to establish a username and password, and begin your usage of the patient portal for personal medical information.

Ways To Enroll:

Before you leave the health center, you will receive an enrollment token to sign up for NextGen Patient Portal. To protect the privacy of

your medical information, this token should be kept secret. You will be required to enter this token to complete your enrollment. Once the

enrollment process is complete, you will no longer need it. Please note that your enrollment token expires after 30 days; if you misplaced

your enrollment token or have not enrolled within the 30 day period, please contact Will County Community Health Center at (815) 727-

8670 and a new enrollment token will be assigned.

By Computer:  Go to the link below and follow the steps.  

  1. Click on “I have an enrollment token.”  Enter your “token” number located on the white piece of paper received from the registration team. The token number expires after 30 days.

  2. Enter date of birth and email address. You can register without an email address.

  3. Choose a username and password. Password must be at least 6 characters and no more than 20 characters. It must also contain at least one number.

  4. Choose one security question from the list and then you create another security question, and enter your answer for those questions.

  5. In case you forget your password, you will be asked to answer the two security questions.

To sign up, log on to and enter your assigned enrollment token number. Once you sign up for NextGen Patient

Portal, you can log in any time at

  1. Go to the app store, and search for Nextgen Patient Portal.

  2. Download the app for free.

  3. Follow steps #1 through #6 under Ways to Enroll: By Computer
