Our Patient Portal allows you online access to your personal health record. The program also allows you to:
- View your test results once your provider has reviewed and released them
- Send non-urgent messages to your care team
- Request medication refills
- Request appointments. If it is your first time on the site, click the blue bar in the box on the top right that reads “I am new here.” You will then be able to establish a user name and password, and begin your usage of the patient portal for personal medical information.
Before you leave the health center, you will receive an enrollment token to sign up for NextGen Patient Portal. To protect the privacy of your medical information, this token should be kept secret. You will be required to enter this token to complete your enrollment. Once the enrollment process is complete, you will no longer need it. Please not that your enrollment token expires after 30 days; if you misplaced your enrollment token or have not enrolled within the 30 day period, please contact Will County Community Health Center at (815) 727-8670 and a new enrollment token will be assigned.
