About BBO-C

better birth outcomes logoBBO-C is a statewide program that assists eligible high-risk pregnant and postpartum clients and newborns in connecting with community services. BBO-C Nurse Navigators provide direct 1:1 assistance to help clients meet their family’s health and wellness goals and promote the reduction of maternal and infant morbidity and mortality.

BBO-C Program Eligibility

Eligibility for BBO-C services is defined as a family/dyad or individual meeting all of the following:

  1. Resident of Will County; and
  2. Pregnant or postpartum individual up to 6 months after birth and/or infant up to 6 months after birth; and
  3. Household income less than 200% Federal Poverty Level (FPL); and
  4. The presence of at least one risk factor:
    * Elevated risk due to previous or current poor birth outcome
    * Elevated risk due to high risk medical diagnosis or social condition of birthing person
    * Elevated risk due to high risk medical diagnosis or social condition of infant

Families not meeting the eligibility criteria may receive administrative pre-approval authorization.

How to Apply

Applicants must live in Will County and meet eligibility requirements to participate. Eligibility can be determined via telephone.
