Program Manager – Epidemiology and Congregate Response – FHS/708 – REPOST – COVID Crisis Grant funded until 6/30/23, then ARPA Grant funded until 12/31/24
Under the direction of the Program Coordinator of Epidemiology and Communicable Disease, provide overall management and support for a unified congregate response to Healthcare Acquired Infections and Multi-Drug Resistant Organisms as required by IDPH.
Communicable Disease Investigator I or II or III – FHS/707 – REPOST – COVID Crisis Grant Funded until 6/30/23, then ARPA Grant Funded until 12/31/24
Candidate will conduct investigation, surveillance, prevention, and control of communicable and non-communicable diseases of public health.
Case Management Administrative Assistant-FHS – ARPA Funded until 12/31/24 – FHS/704 – REPOST
Candidate will perform clerical duties with a high level of data entry and will have a variety of confidential responsibilities while working with protected health information.
Communicable Disease Investigator I, II, or III – FHS/119
Candidate will conduct investigation, surveillance, prevention, and control of communicable and non-communicable diseases of public health.
Nutrition Specialist – FHS/112
Under the supervision of the Division Director through immediate supervisors; plans, organizes and evaluates nutrition component of Women, Infants and Children (WIC). Conducts group education, health assessments and certifies clients eligible for WIC. Performs hemoglobin screenings; assess needs for health and social services; develops an individual care plan in conjunction with clients; periodic assessments, follow-up, and referral activities.

Candidate will conduct investigation, surveillance, prevention, and control of communicable and non-communicable diseases of public health.
Staff Nurse II or III – FHS/701 – ARPA FUNDED UNTIL 12/31/24 – REPOST
Candidate will provide case management services to clients in our Better Birth Outcomes, High Risk Infant Follow-up, Medical Case Management for children at-risk, and lead poisoning prevention programs.
