Responsible to the Division Director of Family Health Services. Coordinates the Community Health Education Programs and responsible for personnel, functions, equipment, and activities as assigned. Manages Community Health Educators.
The Community Health Educator will be responsible for all activities required by assigned Community Health Initiatives/Grants with heavy emphasis on Illinois Tobacco Free Communities (Tobacco Program).
Works under the supervision of the Division/Department Head through immediate supervisor as directed or required.
The candidate will provide immunizations at the main office, and branch offices as assigned. Candidate will also cross train to other Family Health Services programs including Healthworks, Lead Poisoning Prevention, High-Risk Infant follow-up.
The Community Health Educator will be responsible for all activities required by assigned Community Health Initiatives/Grants.
Candidate will conduct investigation, surveillance, prevention, and control of communicable and non-communicable diseases of public health.
