REQUIREMENTS FOR 12-17 YEAR OLD VACCINES

All minors under the age of 18 must have a consent form signed by a parent or legally authorized representative at the time of your vaccine appointment.  It’s simple:

  1. Download the FORM here
  2. Read the FACT SHEET for Recipients and Caregivers
  3. Fill out the form for each minor
  4. Sign and print the consent form to bring to your appointment.

All minors under 18 must have a parent or guardian present during their appointment, and those 12-17 can only receive a Pfizer dose.
