REQUIREMENTS FOR 12-17 YEAR OLD VACCINES
All minors under the age of 18 must have a consent form signed by a parent or legally authorized representative at the time of your vaccine appointment. It’s simple:
- Download the FORM here
- Read the FACT SHEET for Recipients and Caregivers
- Fill out the form for each minor
- Sign and print the consent form to bring to your appointment.
All minors under 18 must have a parent or guardian present during their appointment, and those 12-17 can only receive a Pfizer dose.

The Will County Health Department is leading the charge to give access to the vaccine for residents and workers in our county. In addition to the Health Department sites, we have partnered with local pharmacies and community-based organizations to hold vaccination clinics. Search below for the nearest vaccination clinic.
