Job Summary
Functions as a professional member of the Will County Community Health Center Clinical team. The candidate will perform a variety of functions to support quality healthcare services. Provide acute and chronic direct patient care services while working with psychiatrists and other members of the behavioral health care team. Improve patient access and support the quality initiative process. Schedule Five (5) days per week with one (1) day being late day (10am – 6pm), and one (1) Saturday every six (6) weeks (with compensatory day off during the week).

Minimum Qualifications

MINIMUM QUALIFICATIONS
Graduate of an Accredited Psychiatric Mental Health Nurse Practitioner Program. National Certification as a Psychiatric Mental Health Nurse Practitioner. Current State of Illinois Advanced Practice Nurse, IL Controlled Substance License, DEA License in good standing. Experience in the provision of mental health care to children as well as adults. Current CPR Certification.

SPECIAL REQUIREMENTS
Vivitrol/Suboxone prescribing

REQUIREMENTS
Must possess a valid Illinois Driver’s License and minimum auto liability insurance. Verification of U.S. citizenship or employment eligibility.

DESIRED:
Bilingual in Spanish. Prior experience with care provision in Community Health Care to uninsured and underserved populations. Experience in the provision of substance abuse treatment.
