Provides a full scope of primary and preventative health care services to all adults and children to meet the health care needs of clinic patients. Direct clinical patient care activities to equal 95% and administrative activities to equal 5%.

Provides comprehensive assessments, evaluations, personalized treatment planning, and comprehensive group and individuals therapeutic services to adults and adolescents experiencing life-area impact from the use of substances.
Administrative Clerk I (BOLINGBROOK) – FHS/058
Duties include a high level of data entry, telephone reception, registration scheduling, filing, timekeeping, and using customer service skills. Position is for the WCHD WIC Program. Schedule Monday, Tuesday, Thursday, and Friday, 8:30 am-4:30 pm, and Wednesday, 10:30 am-6:30 pm. Candidate must be able to travel to other sites as needed.

Management level position which provides supervision, training, and guidance to staff in WIC in Family Health Services. This position ensures compliance with federal and state guidelines while overseeing daily clinic program operations.
Staff Nurse II – BH/063
Under the direct supervision of the Program Coordinator, the applicant will participate as a member of an assigned/designated Treatment Team in providing community-based clinical nursing services; intensive outreach, crisis intervention, social rehabilitative, and psycho-educational services for mentally ill children in various settings (on and off-site).

Medical Secretary – BH/042
The candidate will be responsible for medical records, respond to all incoming record requests, send records, and work with third-party vendors to retrieve medical records. Requests will come from various sources (in-person, faxes, email, etc.). Answering incoming calls and making outgoing calls, concerning medical records, is another facet of the position
