Job Summary
Responsible for planning and executing Medical Case Management for existing and newly diagnosed HIV/AIDS patients with high need and high viral load. The role of the case manager is to engage in high-quality efforts to assess, link, and ensure benefits acquisition for their patients. The Case Manager is expected to have a vigorous pursuit of ensuring that Persons Living with HIV/AIDS (PLWA) continue to receive care and treatment services while being informed, educated, and enrolled into coverage systems. Schedule Monday through Friday, 9:00 am – 5:00 pm.
Minimum Qualifications
MINIMUM QUALIFICATIONS
Care Coordinator/Case Manager I: Bachelor of Arts/Bachelor of Science degree from an accredited institution required in social/health sciences, Sociology, or Psychology. Or Associate Degree in Nursing with a valid Illinois RN license AND a Minimum of two (2) years of relevant experience.
Care Coordinator/Case Manager II: Master of Arts/Master of Science, or Bachelor of Science in Nursing degree from an accredited institution required in social/health sciences, Sociology, or Psychology. And a Minimum of two (2) years of relevant experience.
REQUIREMENTS
Must possess a valid Illinois Driver’s License and minimum auto liability insurance. Verification of U.S. citizenship or employment eligibility.
SPECIAL REQUIREMENTS
Knowledge and experience in Microsoft Programs (Word, Access, PowerPoint, and Excel), Adobe, and have database management skills.
DESIRED
Bilingual in Spanish. Prior experience in case management, or community health, or HIV/AIDS population health.
Salary Range
Grade 13 Hourly Wages ($26.64 – $38.59)
Grade 14 Hourly Wages ($29.61 – $42.88)
