Job Summary
Candidate will provide case management services to clients in our Better Birth Outcomes, High-Risk Infant Follow-up, Healthworks/Medical Case Management, and lead poisoning prevention programs. May be crossed trained to assist in other programs (e.g., Immunizations, Sexually Transmitted Infections). Home visits could be required. Schedule, Monday through Friday, 8:30 am – 4:30 pm.

Minimum Qualifications

MINIMUM QUALIFICATIONS:
Staff Nurse II: Accredited Diploma or Associate in Nursing and be a Registered Nurse (RN) by the State of Illinois or reciprocal State required.
Staff Nurse III: Accredited Bachelor’s degree in Nursing and be a Registered Nurse (RN) by the State of Illinois or reciprocal State required.

REQUIREMENTS:
Must possess a valid Illinois Driver’s License with minimum auto liability insurance. Verification of U.S. citizenship or employment eligibility.

DESIRED:
Bi-lingual in Spanish.

Salary Range

Staff Nurse II Grade 13 Hourly Wages ($22.34 – $34.85)

Staff Nurse III Grade 14 Hourly Wages ($24.83 – $38.72)
