Job Summary
Provides direct behavioral health assessments, diagnoses, and therapy; candidate will also provide case management referrals and follow-up services. Duties include, but are not limited to; assesses, diagnoses, and formulates documentation and implementation of treatment plans addressing the mental and behavioral health of patients. Case management is provided through triage, referrals, provider consultations, therapy sessions with individuals/families, and/or groups, and other outside agencies. May provide and manage substance abuse screenings. Schedule, Monday through Friday, 8:00 am-4:00 pm. Social Worker may be required to work one to two days in the Joliet Office.

Minimum Qualifications

MINIMUM QUALIFICATIONS
A Licensed Clinical Social Worker with one (1) year experience. Must possess an Illinois license in good standing.
 
REQUIREMENTS
Verification of U.S. citizenship or employment eligibility. Must possess a valid Illinois Driver’s License with minimum auto liability insurance. Separate background checks will be conducted by the school and Will County Health Department. 
 
DESIRED
Bilingual in Spanish. Certificate of Alcohol and Drug Counseling-CADC.

Salary Range

Grade 14 Hourly Wages ($27.84 – $41.33)
