Job Summary
Functions as a professional member of the Will County Community Health Center Clinical team. Candidate will perform a variety of functions to support quality healthcare services. Provide acute and chronic direct patient care services while working with Primary Care Physicians and other members of the health care team. Improve patient access and supports the quality initiative process. Part of the time may be spent coordinating and delivering primary health care services outside the clinic in the Mobile Health Unit. Ability to work flexible hours and one Saturday every 4-6 weeks. Travel between Joliet, Monee, and Bolingbrook may be asked.

Minimum Qualifications

MINIMUM QUALIFICATIONS

Master’s Degree in a health-related field from an accredited institution. Two (2) years of experience in Family Planning or OB/GYN nursing. Current Illinois State Advanced Nurse Practitioner/Certified Nurse Practitioner License. Certification in CPR and maintaining a current license.

REQUIREMENTS

Verification of U.S. citizenship or employment eligibility. Must possess a valid Illinois Driver’s License with minimum auto liability insurance.

SPECIAL REQUIREMENTS

Full-scope practice of family medicine (newborns to geriatrics).

DESIRED

Bilingual in Spanish.
