Job Summary
Functions as a professional member of the Will County Community Health Center Clinical team. Candidate will perform a variety of functions to support quality health care services. Provide acute and chronic direct patient care services, while working with Primary Care Physicians, and other members of the health care team. Improve patient access and supports the quality initiative process. Schedule, Monday through Friday 8:00 am – 4:00 pm. Must be willing to travel to locations in Bolingbrook, Joliet, and Monee.

Minimum Qualifications

MINIMUM QUALIFICATIONS:
U. S. graduate of an Accredited Psychiatric Mental Health Nurse Practitioner Program. National Certification as a Psychiatric Mental Health Nurse Practitioner. Current State of Illinois Advanced Practice Nurse, IL Controlled Substance License, DEA License in good standing. Two (2) years’ experience as a nurse in a clinical setting. Experience in the provision of mental health care to children as well as adults. Current CPR Certification.

SPECIAL REQUIREMENTS: 
Vivitrol/Suboxone prescribing (may obtain suboxone certificate after starting).

REQUIREMENTS: 
Must possess a valid Illinois Driver’s License and minimum auto liability insurance. Verification of U.S. citizenship or employment eligibility.

DESIRED:
Bilingual in Spanish. Prior experience with care provision in Community Health Care to uninsured and underserved populations. Experience in the provision of substance abuse treatment.
