The Community Health Educator will be responsible for all activities required by assigned Community Health Initiatives/Grants.
Candidate will perform clerical duties with a high level of data entry with completing Patient Registration by providing information regarding registration and eligibility process; receiving, verifying, and entering data.
Candidate will provide case management services to clients in our Better Birth Outcomes, High Risk Infant Follow-up, Medical Case Management for children at-risk, and lead poisoning prevention programs.

Under the direction of the Program Coordinator of Epidemiology and Communicable Disease, provide overall management and support for a unified congregate response to Healthcare Acquired Infections and Multi-Drug Resistant Organisms as required by IDPH.
Candidate will conduct investigation, surveillance, prevention, and control of communicable and non-communicable diseases of public health.
